PUBLIC RECORDS REQUEST
Requests for public records can be made in person at the Little Chute Safety Center, by telephone (920) 788-7505, by fax (920) 788-7385, by e-mail to Telecommunicators@fvmpd.org or US mail. To facilitate your request, complete the Request for Public Records form available from this website or at the department’s lobby service window.
Typical requests are for incident reports, record checks, digital audio, digital video tapes and digital images. There is a charge for copies of these records:
The department will charge a fee for the actual, necessary, and direct costs of reproduction of records. Permissible charges include:
- Copying and reproduction
- Location, if exceeds $50.00
- Mail and shipping
- Computer programming needed to location and produce records
- Other specified in 19.35(3) Wis. Statutes
- Photo copies $0.25 per page
- Video files $20.00 per disc
- Images $2.00 per printed page, $20.00 per disc
- Audio files $20.00 per disc
- Additional fess apply if mailed, Postage 1st Class Rate
All requests for copies of crash reports investigated by the department will be fulfilled by the State of Wisconsin Department of Transportation through the State’s online portal at http://wisconsindot.gov/Pages/online-srvcs/external/dmv.aspx The cost for a crash reports is $6.00 each, payable by Credit, Debit card, or ACH. Reports are available two (2) business days after they are received by the Wisconsin Department of Transportation.
STEP BY STEP INSTRUCTIONS FOR OBTAINING A CRASH REPORT
Fingerprints-Can be obtained at the Kaukauna Police Department or Grand Chute Police Department by appointment
Payment is due upon the receipt of the records. Fees in excess of $5.00 will require prepayment.
The department shall respond to records requests as soon as practicable, as outlined in §19.35(4) (a), Wis. Stats. Absent exceptional circumstances, a request should be fulfilled within 10 business days. Should a request take longer to fulfill, the requestor shall be notified of the reason for the delay and provided with an estimated date of completion. The actual, necessary, and direct costs of reproduction of records shall be assessed.

The Telecommunicators are available from 8:00 A.M. to 4:30 P.M. Monday through Friday.

To obtain Driver's License records or Vehicle Registration records, a Vehicle/Driver Record Information Request form must be completed and mailed to:

To obtain criminal history records, a Criminal History Record request form must be completed and mailed to:
